Project Call: Are you a health professional? We need your help to be part of a pilot program to reduce hospital admissions in older people

The Plug-in, the University of Adelaide Nursing School, Adelaide PHN and Central Adelaide Local Health Network are working to improve the provision of integrated care for older people. We have previously consulted with older people, health professionals and service providers who have told us that ‘There is something missing in the middle’.

We now want to address this problem in a project that is supported by funding from The Hospital Research Foundation.

We want to hear from health professionals and service providers in the north-east and south-west suburbs of Adelaide to understand the needs of older people aged over 75 during transitions of care between hospital and the community. This will help us to develop and test a solution for a more effective and person-centred approach to integrated care.

This project will help us to investigate what type of person/position would help provide continuity of care when moving between care from GP’s, hospital and at home.

The aim of this project is to reduce multiple admissions to hospital and to help older people remain independent and living well at home!

What’s involved?

To hear your insights and experiences we will be holding workshops to explore ideas for a more effective and person-centred approach to integrated care.

The workshops will be 90 minutes in duration. If you are selected to take part you are entitled to receive a $240 payment for your participation.

Light refreshments will also be provided at the workshop.

The workshop times and locations are:

  • North-east Adelaide (venue TBC): 6.30 – 8pm, Wednesday October 30th 2019
  • South-west Adelaide (venue TBC): 6.30 – 8pm, Wednesday November 6th 2019

Am I eligible?

As with all our projects, The Plug-in has a range of selection criteria for participants.

We need health professionals + service providers who:

  1. Work or operate in either the north-east or south-west suburbs of Adelaide
  2. Who provide primary health care, community care and/or acute care services for older people
  3. Are able to attend one of the 90 minute workshops listed above
REGISTER YOUR INTEREST
