  2. You must be a resident of Australia to eligible for The Plug-in’s “Health and aged care research findings survey” Promotion.
  3. All employees of the COTA Federation, members of associated governing bodies, their family members and individual aged under 50 are ineligible for the Promotion.
  4. The Promotion commences on 6 February 2020 and closes 23:59 ACST on 5 March 2020. Registrations received after the close date or who are ineligible will not be considered for the Promotion.
  5. Cost of entry is free, and entry into this Promotion is deemed acceptance of these Terms and Conditions.
  6. To enter, an individual must complete The Plug-in’s “Health and aged care research findings survey”.
  7. An individual can only register once. Entries on behalf of another person will not be accepted, and joint submissions are not permitted.
  8. The Plug-in shall use and take care of any personal information supplied as in accordance to with the COTA SA Privacy Policy link. By entering the Promotion, an individual agrees to the collection, retention and usage of their personal information in order to process your entry, and for the purposes outlined in these terms and conditions.
  9. The Plug-in accepts no responsibility for entries that are lost, delayed, misdirected or incomplete or cannot be submitted for any technical or other reason.
  10. The winners will be determined by way of a random electronic draw from all entries received in accordance with these Terms and Conditions. The draw will take place at 12 pm on Wednesday 11 March 2020 at COTA SA, 16 Hutt Street, Adelaide, 5000.
  11. Five winners will receive a $30 gift card. The prize is non-transferable and cannot be exchanged or taken for cash and can be collected from COTA SA unless alternative arrangements are negotiated. The Plug-in accepts no responsibility for any costs associated with the prize that are not explicitly included in the Terms and Conditions..
  12. The winners will be notified by phone and published on The Plug-in website on Thursday 12 March 2020. If a winner rejects their prize, fails to respond within 14 days of notification or the entry in breach of these Terms, then the winner’s prize will be released, and The Plug-in shall be entitled to draw another winner.
  13. The Plug-in accepts no responsibility for any damage, loss, injury or disappointment incurred or suffered as a result of entering the promotion or accepting the prize. The Plug-in disclaims liability for any injury or damage to any individual or technical device resulting from participation in connection with the Promotion.
