Have you thought about who you would like to make decisions about your future health care in the event you are unable to?

We want to hear from people who are considering an Advance Care Directive, have completed one, and generally interested in knowing more. There is no need to have prior knowledge on this topic, just a desire to be involved and help shape a key government document so it is easy to understand.

The Plug-in has partnered with SA Health to understand community experience of Advance Care Directives — a legally recognised document that provides direction about your wishes, preferences and instructions for future health care.

Following extensive consultation and feedback, SA Health has been revising the existing Advance Care Directive form and DIY Kit to be easier to read, access and complete. Before it is finalised, we want your feedback on the readability of the new form and guide – you could be one of the first people to see the new design and influence the final product, which will help other people like you!

To be eligible for this project, you just need to be aged 50+ and able to attend a focus group in Adelaide.

What’s involved?

The Plug-in is hosting a series of focus groups in metropolitan Adelaide to test and explore the revised Advance Care Directive form and guide. Focus groups will be run in early June 2022, and you can find out more details and register your interest to take by completing the survey at the link below.
